Position Summary:  The Service Providers (SPs) are primarily responsible for delivering quality Children & Family Treatment & Support Services (CFTSS) and/or Home and Community Based Services (HCBS) services to eligible youth to support the achievement of safety, well-being and permanency objectives.

CFTSS and HCBS are Medicaid funded services that are designed to allow children/youth ages 0-21 who are at risk of developing or have been diagnosed with behavioral, developmental or medical diagnoses to participate in a vast array of developmentally and culturally appropriate services in the home and in the community.

The position will include providing one or more of the following services:

  • Community Psychiatric Support & Treatment - services works to develop child’s social and independent living skills, stabilize child and family after crisis and help caregivers respond in situations that could trigger a crisis. Support services work with caregiver to plan child’s treatment educate child on appropriate safety and self-care, and connect to resources. 
  • Psycho-Social Rehabilitation - assist with study habits, coping skills, hygiene, daily living activities and socialization for a child with mental health needs they can address inappropriate behaviors, anger management and coping mechanisms. 
  • Caregiver/Family Support Services - enhance child’s ability to function as part of a family/ caregiver unit and enhance the family/ caregiver’s ability to care for the child receiving services. 
  • Planned Respite– provides planned short-term relief for families’ caregivers that are needed to enhance the family caregiver ability to support the child’s disability and or health care issues. 
  • Community Self Advocacy Training and Supports - improve the child’s ability to gain from the educational experience of attending school, participating in community activities and enable the child’s school/community to respond appropriately to the child’s disability and/or health care issues.  
  • Community Habilitation - works one on one with developmentally disabled child to set goals and master skills they need at home and in the community including hygiene coping skills daily living activities like chores and budgeting getting around the community and socialization.  
  • Prevocational Provider- helps teens 14+ with topics such as proper work habits problem-solving resume preparation and interviewing. 
  • Supported Employment - provides teens 14+ with a job coach to assist in finding a job or vocational opportunities. When appropriate the provider can also work with an employer in order to make the experience rewarding for everyone involved.   

Essential Functions:

  • Maintain assigned schedule; ensure frequency, scope and duration of services is met
  • Face to Face services required upon request or as per client need
  • Flexible hours, including evenings and/or weekends
  • Attend supervision by Administrative team, including regular supervision by a LCSW
  • Maintain contact with collaborating Care Coordinators, Case Managers and CFTSS Intake team
  • Review and complete the treatment plans related to the services they are providing in a timely fashion.
  • Monitor the youth’s progress in achieving goals detailed in the treatment plan.
  • Collaborate with other care providers and community supports to help youth meet their needs
  • Complete Progress Notes after every service timely in the Electronic Health Record, no later than 48 hours of the event
  • Complete contact logs for any non-billable client related encounters
  • All documentation must follow agency policies in regards to completion and timeliness.  

Qualifications:

  • Bachelor’s degree and three years applicable experience in children’s mental health, addiction, and/or foster care/child welfare/juvenile justice or another related human services field

Hourly Range: $26.83-$27.81 per hour
