POSITION DUTIES:

1. Assists the Director in the development, implementation and monitoring of program systems to ensure quality, quantity and documentation of services provided in compliance with agency policy as well as state and federal regulations.

2. Ensures integration with residential providers. Oversees coordination of meetings with the people who attend the program residences and Care Managers to ensure the development and implementation of coordinated person centered planning to meet compliance regulations and timeframes.

3. Ensures that all individualized programs reflect independence, individuality, inclusion, productivity and opportunities to learn new skills that will enable the people attending our program to meet the challenges in work related environments and in daily activities.

4. Provides supervision, direction and training to all staff members in program development, implementation, monitoring and person center planning.

5. Works with the center’s management team to coordinate and monitor service delivery for the people who attend the program.

6. Schedules and coordinates trainings to ensure compliance with agency policy as well as state and federal regulations and timeframes.

7. Implements, coordinates and reviews policies and procedures ensuring compliance with state, federal and agency regulations.

8. Reviews staff utilization to ensure the provisions of appropriate staffing ratios in the classrooms.

9. Interviews, hires, evaluates, monitors, trains and disciplines staff.

10. Reviews and certifies all Staff Action Plans, Day Habilitation Plans, Reviews and Monthly Notes.

11. Ensures that group room schedules are adhered to and that participants are engaged in

meaningful, appropriate activities.

12. Performs all other duties as assigned.

C. REQUIREMENTS:

1. B.A/B.S. degree in a related field.

2. Thorough knowledge of day habilitation (690) regulations.

3. Minimum 3 years supervisory/management experience.

4. QIDP status.

5. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and Federal False Claims Acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.

6. Knowledge of the Precision Care Application.
