
The primary role of the Community Habilitation Specialist is to work alongside the people we support and provide Medicaid Waiver Services in community-based settings in order to help them to achieve their Personal Outcomes.
The primary role of the Community Habilitation Specialist is to work alongside the people we support and provide Medicaid Waiver Services in community-based settings in order to help them to achieve their Personal Outcomes. This includes working with individuals with intellectual and developmental disabilities to develop daily living, professional, and decision-making skills while also supporting their health and safety.
Position Duties:
Provides training in accordance with valued outcomes specified by each specific Staff Action Plan and Life Plan.
Provides training and supervision based on participant’s abilities including areas of self-care, independent living, community integration, work-related skills development, etc.
Ensures the safety and security of the person receiving services by taking into account their specific needs and limitations. Direct services that support the participant(s) in the least restrictive manner.
Ensures that each person is receiving services in the least restrictive manner, while working on their PCP goals to obtain greater independence
Completes all documentation for service delivery in accordance with federal and state regulations.
Provides timely services that are in accordance with each participant’s set schedule to ensure consistency and the respect of each person receiving services.
Works collaboratively with the team that supports the person(s) receiving services.
Responds appropriately to crisis/emergency situations in order to secure the safety and well-being of participant(s).
Promotes program development by actively seeking participants in need of services.
