Cerebral Palsy Transport, Inc. Drug Policy

Cerebral Palsy Transport, Inc. is committed to provide its employees with a safe work environment and promote programs which encourage high standards of employee health. For their part all employee are expected to be in suitable mental and physical condition while at work, and to perform their jobs in a satisfactory fashion. In instances in which the use of alcohol or other drugs interferes with these goals, appropriate action will be taken.

The possession, sale or use of illegal drugs obtained without prescription is inconsistent with our company's objective of operating in a safe and efficient manner. Accordingly, no officer or employee shall use or have such items in his or her possession during working hours or on company property at any time. Additionally, no officer or employee shall report to work while under the influence of such drugs. Employees who engage in such conduct will be subject to discipline up to and including discharge.

All applicants considered for employment will be screened for drugs. Applicants will be requested to sign a consent release form authorizing the drug screening and the submission of its results to our company. Applicants who refuse to sign a consent release form or who test positive for illegal drugs will not be considered for employment.

That I have read and understand its contents and understand that I must be drug free as a condition of employment. Cerebral Palsy Transport Inc. has established an anti-drug program for the purpose of maintaining a drug free work place, in accordance with certain federal regulations and/or company policy. All existing Cerebral Palsy Transport drivers and new applicants for company employment must be drug free in accordance with the company's drug free work place program.

I acknowledge that Cerebral Palsy Transport is vitally concerned with any situation where the use of illegal drugs, or the illegal use of prescription drugs, seriously interferes with any persons health and job performance.
