By signing below, I am authorizing Tillamook County Creamery Association (“the Company”) to obtain complete information and records from any of the persons or organizations named in this application, and I agree to sign a release of liability for all such persons regarding the information provided.

2. I understand that if I am offered employment, I may be required to pass a pre-employment physical as a condition of being hired, consistent with applicable laws. I also understand that I will be required to submit to testing for the presence of unlawful drugs, including Marijuana which remains illegal under federal law, as a condition of being hired and that a positive test result will disqualify me for employment consistent with applicable laws. I further understand that I may be required to agree to criminal history check as a condition of being hired, and that the results must be satisfactory to the Company consistent with applicable laws.

3. I understand that if I am hired I will be responsible for complying with all policies and rules of the Company as they presently exist or are later modified, and I understand that my employment can be terminated at-will, for any reason and at any time without notice, at the option of the employer or myself, except as prohibited by law.

4. I also understand that no representative of the employer has any authority to change my at-will employment status or to otherwise enter into any employment agreement for any specified period of time, or to assure me of any future position, benefits, or terms and conditions of employment, except as specifically stated in a written agreement signed and dated by the Company’s President and CEO.
