Description

Hudson River Healthcare provides comprehensive, affordable, quality primary and preventive care to every patient regardless of ability to pay. AmeriCorps members at Hudson River Healthcare will serve in specific roles to support this effort by engaging community members in activities that (1) Increase Access to Care: through evidence-based and culturally tailored interventions to increase access to and promote the proper utilization of primary and preventive health care services, health insurance, and benefits programs; (2) Support Patients’ Transitions of Care: facilitate transitions of care for patients from hospital-based to primary care settings, improving patients’ and community health outcomes; (3) Deliver Youth Development Programming: delivering a range of health education, nutrition education, and other related programming to encourage healthy behaviors and increase health literacy among community youth in a variety of settings, including among urban and rural youth, and the families of seasonal and migrant farmworkers; and (4) Coordinate Research Endeavors: playing a key role in supporting the development and implementation of research-driven processes that empower patients to become active participants in their health and the health of their families and community. Hudson River Healthcare is partnered with CHCANYS in seeking AmeriCorps Members for its 2020 CHCANYS Healthy Futures Corps Program.

AmeriCorps members at Hudson River Healthcare will serve in specific roles to support this effort by engaging community members in activities that (1) Increase Access to Care; (2) Support Patients’ Transitions of Care; (3) Deliver Youth Development Programming; and (4) Coordinate Research Endeavors. AmeriCorps members do not provide health services related to prohibited items or activities as is outlined in detail during orientation. 

Minimum Education Requirement: Some college. Must possess high school diploma/GED *Must be over the age of 18.

Program Benefits: Education award upon successful completion of service, Living Allowance, Childcare assistance if eligible, Health Coverage and Training. 

Terms: Permits attendance at school during off hours and working at another job during off hours
