SUMMARY:

Assists physicians and nursing staff in management of patients with internal reservoirs, fistulas, draining wounds, special wound and skin problems, pressure ulcers, skin ulcers, and incontinence.  Assesses, plans, implements, evaluates and documents enterostomal patient education needs and care according to established hospital policies.  Provides in-service training to hospital staff.  Demonstrates specific care needs based on knowledge of correct prosthetic ostomy equipment and related products.  Promotes patient education and care by developing and updating programs.

 

ESSENTIAL FUNCTIONS:

Demonstrates patient care procedures and encourages patients to develop confidence with daily routines and technique s for temporary and permanent ostomies of all kinds, and for management of incontinence.

Recognizes signs of potential post-operative complications and determines appropriate course of action to minimize further trauma to the patient.

Prepares and conducts patient consultations, training and in-service education for staff.

Ensures accurate maintenance of patient records and treats patient records as confidential information.

Facilitate communication of patients’ wound management care plan to receiving SNF or home care agency.  Provide efficient patient discharge with appropriate patient and family education for wound care management.

Develops and discusses proposed care plans with patient’s attending physician.

Develops, implements and administers in-service education for nursing staff; formulates and develops plans, procedures and programs to meet specific training needs and problems.

Ensures the educational needs of staff are met; acts as an education resource to nursing community and health care team; d evelops referral sources and community education programs.

Participates in QI (Quality Improvement) programs on a regular and continuing basis.

Collects, aggregates, displays and conducts first level analysis of data obtained from monitoring and evaluating the quality or utilization level of patient care.

Assists in research and development directly related to enterostomal therapy services.

Provide oversight of appropriate utilization of negative pressure therapy. Monitors and reduces equipment rental fees, provides efficiency in dressing changes and utilization of appropriate dressings, assists in the reduction of wasted supplies.  Assists all nursing units with training, education and utilization of this technology as requested.

Investigates new products, equipment and techniques; makes recommendations to nursing director.  Participates in demonstration of these to department staff.

Facilitates required vendor documentation for equipment use post discharge to home or another facility.

Implements and maintains established department, unit and hospital policies, procedures and objectives as well as quality assurance, environmental and infection control policies.

Adheres to TMC organizational and department-specific safety, confidentiality, values policies and standards.

Performs related duties as assigned.

 

MINIMUM QUALIFICATIONS

 

EDUCATION:  Bachelor’s degree in nursing (BSN); or equivalent combination of relevant education and work experience in wound/ostomy practice.

 

EXPERIENCE:  Four (4) years of wound/ostomy experience.

 

LICENSURE OR CERTIFICATION:  Current RN licensure permitting work in the State of Arizona, or Compact State.  Certified in or attending an approved training program for certification in Wound, Ostomy, and Continence Nursing. BLS/CPR certification.
