Prior to commencing employment, prospective employees are required to satisfactorily complete all required pre-employment activities, including a health screen.

I certify that this application was completed by me and that the answers to the questions on this application are true, accurate, and complete. I understand that any misrepresentation or omission made as part of this application may be considered sufficient cause for denial of employment, revocation of any employment offer I may receive, or immediate termination if discovered after my employment begins, irrespective of the time lapsed before discovery.

I understand that any offer of employment from New England Baptist Hospital is contingent upon satisfactory completion of a pre-employment physical examination conducted by New England Baptist Hospital which includes post offer drug and nicotine/cotinine testing. I further understand that any offer of employment by New England Baptist Hospital is contingent upon verification of my identity and eligibility to work for New England Baptist Hospital. I understand that NEBH is a tobacco free facility and no smoking is allowed within any building or at any place on NEBH's property. I also understand that the use of illegal drugs is prohibited during employment. If the company requires, I am willing to submit to drug tests to detect the use of illegal drugs during my employment.

I understand that if I am hired by New England Baptist Hospital, I will be an employee "at will" and may be terminated at any time, with or without cause, and with or without notice. I also understand that no representative of New England Baptist Hospital, other than the President by a written signed contract, has the authority to enter into any agreement with me for employment for any specified period of time or to make any agreement contrary to the foregoing.

If I am hired by New England Baptist Hospital, I agree to abide by its policies, rules and procedures and any changes thereto.

I consent moreover, that if I am hired by New England Baptist Hospital, I will uphold and comply with the following:

New England Baptist Hospital's Dress to Impress policy

The expectation for all uniformed and non-uniformed employees, students, volunteers, physicians, and vendors is to adhere to the following grooming and clothing standards. The respectful and professional image New England Baptist Hospital wants to present to our patients and visitors requires that all employees, students, volunteers, physicians, and vendors dress in professional business attire, at all times.

  • It is the expectation that all managers, directors and vice presidents will wear a blazer, jacket, cardigan or white lab coat during their shift.
  • Professional attire requires a clean, neat, and business-like appearance.  Attire such as shorts, t-shirts with pictures or writing, and athletic wear is not professional.  Similarly, dresses and skirts/skorts should be in appropriate, business-like style and length.  Mini-skirts and tank dresses are not appropriate.  Generally, clothing that is revealing or tight fitting is not professional and is, therefore, inappropriate.
  • Clothing should be clean and pressed, not wrinkled, torn, faded or frayed.  Fabric such as denim, sweat-jersey, Lycra, and leather are not appropriate.
  • Hair is clean and neatly combed.  Extreme or eccentric hairstyles and colors are inappropriate.
  • Men are clean-shaven or have a neatly trimmed mustache or beard.
  • Makeup and or perfume/after-shave should be appropriate and not overdone.
  • Nails should be clean and trimmed to a professional length that does not interfere with an employee’s work or patient safety.  Any unusual/bold nail polish colors [i.e., green, blue, yellow, purple, black, etc.] or nail art/jewelry is not appropriate.

Providers and Indirect Providers of Patient Care

  • Long natural fingernails and artificial fingernails are not be worn
  • Natural fingernails should be of reasonable length (1/8 inch beyond fingertip and well manicured).
  • Fingernail polish may be worn only if completely intact.  Chipped fingernail polish must be removed.
  • Jewelry should be professional and not overdone, and should not interfere with an employee’s work or safety.
  • Shoes and briefcases/tote bags should be cleaned/shined and in good condition.

NEBH's Employee Dependability Policy

The ability of New England Baptist Hospital (NEBH) to provide outstanding patient care and legendary service is dependent on employees reporting to work on time and as scheduled. While compelling reasons sometimes necessitate absence or tardiness from work, excessive unscheduled absence and/or tardiness is detrimental to NEBH's operations, places unfair burden on co-workers and will not be tolerated. Departments may create and require that their employees follow policies that are more specific and more restrictive than the general policies outlined below, as long as such policies have been reviewed and approved by Human Resources.

  • The term "absences" in this policy does not include any absences that are legally protected (for example, FMLA, SNLA, MMLA or reasonable accommodation leaves) or otherwise consistent with other NEBH policies such as bereavement leave.
  • Tardiness: Punctuality is essential to the successful operation of NEBH. Tardiness disrupts schedules, adds overtime costs, and places additional responsibilities on the employees in the department. Employees are expected in their work area ready to work at the designated start time. If a late arrival is unavoidable, the employee’s supervisor must be notified as soon as possible. Excessive tardiness and/or staying away from work without proper notification will result in corrective action up to and including termination of employment.
  • Attendance: Regular attendance is essential to the successful operation of NEBH. This policy defines attendance expectations as no more than five incidences of unplanned absences and/or tardiness in a twelve-month period (for example, three absences could equal one incident). Excessive absenteeism and/or staying away from work without proper notification will result in corrective action up to and including termination of employment.

I give New England Baptist Hospital and its agents permission to investigate my references and background to ascertain any and all information of concern to this application, whether the same is of record or not, and I hereby release New England Baptist Hospital from any and all liability, damages, or responsibility from investigating and/or receiving the requested information. I also hereby release my current or former employers and any persons named herein from any and all liability, damages or responsibility on account of furnishing the requested information.

It is unlawful in Massachusetts to require or administer a lie detector test as a condition of employment or continued employment. An employer who violates this law shall be subject to criminal penalties and civil liability.
