SUMMARY:
Coordinates and provides spiritual care to patients, their loved ones, and TMC staff.
ESSENTIAL FUNCTIONS:
Participates in delivering and arranging appropriate spiritual care to patients, their loved ones, and staff.
Responds to requests for spiritual care in a timely manner.
Documents spiritual care visits in patient’s EMR.
Promotes individual spiritual growth and provides a nurturing community for colleagues and volunteers.
Participates in the offering of and meeting ritual and sacramental needs of patients, their loved ones and staff.
Provide spiritual care in an interfaith/ecumenical setting.
Supports an interdisciplinary approach to spiritual care.
Participates in team meetings and rounds.
Serves on institutional committees as appointed.
Participates in maintenance of proper administrative procedures for the department in accordance with institutional policy.
Assists in the coordination of clergy and spiritual care visitors from the patient’s religious community, as requested.
Keeps records of spiritual care activity to communicate with other members of the department and report to department/institution.
Participates in the department’s plan for continuous quality improvement.
Participate in programs, administrative activities of the institution, and regular staff meetings.
Participate in regular professional development activities and supervision.
Adheres to and supports team members in exhibiting TMCH values of integrity, community, compassion, and dedication.
Adheres to TMCH organizational and department-specific safety and confidentiality policies and standards.
Performs related duties as assigned.
EDUCATION: Master’s degree in Theology, or an equivalent combination of relevant education and experience, required. At least one unit of CPE (Clinical Pastoral Education) preferred.
EXPERIENCE: Two (2) years of chaplain or spiritual care experience required. Four (4) years of chaplain or spiritual care experience preferred.
LICENSURE OR CERTIFICATION: Ordination or commissioning for chaplaincy by a recognized religious organization is required. Basic Life Support (BLS) certification and Valid AZ fingerprint clearance card is required for Hospice Chaplains.
KNOWLEDGE, SKILLS AND ABILITIES:
